Transition Planning For Young People With Complex Needs

This 64-page publication by the CSCI on developing better transition planning for young people with complex needs recommends local authorities and Primary Care Trusts (PCTs) to work together to develop seamless services for disabled young people. ‘Complex needs’ includes combinations of acute and chronic medical conditions, multiple and profound impairments, behavioural problems and learning difficulties.
